Community Readiness Assessment Tools

Segment 3 provides an explanation of all the tools necessary to perform a community readiness assessment using the Tri-Ethnic Community Readiness Model. The tool included in this segment are a planning tool, a preparing key respondents document, an interview script, an anchored rating scale, an individual scoring sheet, and a combined scoring sheet.
