After being funded during SFY17 as a PFS Community Readiness Assessment (CRA) Mini-grantee, Carroll was funded as a sub-recipient 8/28/2017. As a CRA Mini-grantee, the community selected a priority substance and priority population based on a review of community-level data and completed a community readiness assessment using the Tri-Ethnic Model. Once funded as a sub-recipient, the coalition collected the required Community Outcomes for FFY17, FFY18, and FFY19 using the statewide youth survey. The sub-recipient conducted listening sessions in order to identify local conditions that impact their intervening variables. The sub-recipient’s strategic plan and media campaign was approved by the Evidence-Based Practices Workgroup. The coalition implemented a media campaign, community-based process and created a problem identification and referral plan. Sub-recipient completed a process evaluation and a sustainability plan.
